By signing this form you understand that Human Balance (H:B) as a subsidiary of Quintessence Limited, Registration no. 06859275, is not registered as a medical practitioner and that they will be providing simple exercises as a well-being technology solution trial.

You agree that no action will be taken against Quintessence should you have any mitigating circumstances during or after the trial period and have no reason to believe you have any medical conditions that would put yourself and Quintessence at risk.

Correction in balance is not intended to diagnose, treat, cure, or prevent any disease or medical condition. Users should always consult with a healthcare provider before making any changes to their exercise routine.

Your data will be held securely and confidentially and not shared with 3rd parties under this agreement. Your data may be used anonymously in the advancement of AI.
